Thousands of breast cancer patients may be safely spared gruelling chemotherapy following a landmark study.
"Every day, women with certain types of breast cancer face the awful dilemma of whether or not to have the treatment, without hard facts about the benefit for them".
Simon Vincent, director of research at Breast Cancer Now, told The Guardian: "This is a remarkable and extremely promising result, but we need to see this effect repeated in other patients before giving hope of a new immunotherapy for incurable metastatic breast cancer".
Watch the full story above. Women in the intermediate recurrence score group were randomly assigned to receive either endocrine therapy plus chemotherapy or endocrine therapy alone. Until now, existing immunotherapy drugs have shown little or no effectiveness against some of the most common cancers, including malignancies of the breast, prostate and colon. After that, a Phase 3 trial will need to broaden the volume of patients treated to verify any positive results, . They were then infused back into the patient.
"Now with these genomic tests, we are finding that we have multiple types of breast cancer, perhaps several dozen", said Brawley, "and we are being able to tailor our therapies to the type of breast cancer every woman has".
The test is performed on tumor samples after surgery, to determine if chemo would benefit a patient.
The decades' long study found that women with the most common types of breast cancer can avoid chemotherapy. Following this "recalibration" of the genetic test, the researchers estimated that chemotherapy was not justified in 70% of cases of breast cancer. Women with an RS of 10 or lower (1619, 17%) were in the ET arm and those with a RS of 26 or higher (1389, 14%) were in the CET arm.
Billions of T-Cells Clear Woman's Breast Cancer in Study
These results provide assurance that getting gene tested is a valuable first step for patients with this type of breast cancer. Kathy Albain, chair of oncology research at Loyola University School of Medicine and lead researcher of the study, said.
Dr. Joseph Sparano of Montefiore Medical Center in NY, lead author of the study says that the impact of this test could spare thousands of women from chemotherapy and for them surgery and hormone therapy could be sufficient. Of the 10,273 women tested, 6,711 (69%) had a score of 11-25.
The cancer in question is driven by hormones, has not spread to the lymph nodes and doesn't contain a protein called HER2.
"The 9-year rate of distant recurrence in women with a recurrence score of 11 to 25 in our trial was approximately 5%, irrespective of chemotherapy use", the authors write.
U.S. Food and Drug Administration Commissioner Scott Gottlieb said he expects the agency will still decide in some cases whether terminally ill patients can receive unproven drugs, despite a new law meant to weaken the FDA's power over such cases.
Perkins said that she could feel the tumors shrinking within the first week of the white blood cells having been pumped into her body.
The study was funded by the National Cancer Institute, some foundations and proceeds from the USA breast cancer postage stamp. The challenge so far in cancer immunotherapy is it tends to work spectacularly for some patients, but the majority do not benefit.
In a very different type of immunotherapycalled CAR-T therapy, a patient's T-cells, the warriors of the immune system, are removed, and then genetically reengineered in a lab to better recognize the patient's own cancer. Scientists are certainly on the right track to finding an effective way to battle cancer, but a lot more work lies ahead to verify these treatments are safe and effective for a large number of people.
