The device works by transmitting a message from the pill to a wearable patch, which then sends the data to a smartphone app, so that patients can keep track of their medication intake personally.

Would you ingest a digital pill that allows you to conveniently track when you took the medication - and possibly allow others to track it as well? By allowing physicians to track a patient's use, Nath said he hopes to avert "dramatic and immediate health-care crises, such as for schizophrenia patients where missing medicines can result in a psychotic break which will land them in an ER".

"The FDA supports the development and use of new technologies in prescription drugs, and is committed to working with companies to understand how technology can benefit patients and prescribers", said Mitchell Mathis of the FDA's Center for FDA Drug Evaluation and Research.

The FDA approved medication, Abilify MyCite, or aripiprazole tablets with a sensor described as the "size of a grain of sand", has been approved for the treatment of schizophrenia, acute treatment of manic and mixed episodes associated with bipolar I disorder and for use as an add-on treatment for depression in adults.

"It is important to note that Abilify MyCite's prescribing information (labeling) notes that the ability of the product to improve patient compliance with their treatment regimen has not been shown", the FDA said in the November 13 statement.

Choudhry, whose work includes developing predictive analytics initiatives to spot patients who don't take their meds, estimates that almost $300 billion is spent in the US each year in "caring for health conditions that are attributable to nonadherence". "Until now, pharmacologic therapy for serious mental illness has been missing a systematic approach to objectively track and signal that a patient has taken their drug". Sometimes the system might not detect that the medication has been taken.

Others have raised concerns about privacy and physician-patient trust, however. "Patients can discontinue sharing some information from the system, or opt out of the program altogether, at any time". Otsuka collaborated with Proteus Digital Health, the Silicon Valley company that designed the futuristic sensor technology.

In the case of Abilify, a patient who doesn't stick to the drug could end up in a psychiatric hospital, so Abilify MyCite may find its niche.

Andrew Thompson, President and CEO of Proteus Digital Health, says, "The time is right for the category of digital medicines to be available to appropriate patients with serious mental illness".

The two companies had tried for FDA approval before, in 2016, but the agency denied it, pending more tests for safety and efficacy.
