The episode intensified scrutiny of New York's antiquated subway system - one of the world's busiest, with more than 5.6 million average daily trips - which has drawn the ire of riders as delays soar and incidents build up. Thirty-four people suffered minor injuries.

The accident occurred about 10 a.m. local time on route A of the NY subway with a train going South. Delays were reported throughout the subway system.

The derailment led to major disruptions and delays Tuesday morning.

The derailment sent travelers tumbling to the floor and forced hundreds of passengers to evacuate through darkened tunnels.

One of the passengers on the train, Medina Browne, said: "At first we didn't know what was happening so it was just really an irritant, just an inconvenience".

The Fire Department New York (FDNY) said the smoke was caused by rubbish setting on fire. "People were falling", said passenger Susan Pak, of Englewood Cliffs, New Jersey.

Lhota said the emergency braking system on the train triggered, but it wasn't immediately clear why.

Transit officials say a preliminary investigation shows that the derailment was caused by an improperly secured piece of replacement rail that was on the tracks.

The Riders Alliance, an organization of subway and bus riders in NY, blamed the derailment on the state's governor, Andrew Cuomo.

"This, to the best of my knowledge, does not look like a failure on the part of equipment, does not look like a failure on the part of the track itself", he said.
